EOC can be contacted 24 hours and day, 7 days a week, at 301-583-2200.

This form or "ticket" is to be used to notify the department of any maintenance and/or damage issues pertaining to the firehouse building(s) and grounds. All issues must not only be reported on this "ticket", but also reported to the PGFD Emergency Operations Center (EOC), with some exceptions. If you have any questions on whether the issue should be reported to the EOC, please contact the President at 240-304-1156 or by email at [email protected]..

If possible, please photograph the damage and email the photographs to the President at pre[email protected] Please specify in the email subject line what the photographs are related to.

  • Specific Information

  • Only one item per ticket. If you have multiple items, please complete separate tickets for each item so we can better track them.
  • All maintenance issues must be reported to the EOC. The EOC can be contact 24/7 at (301)583-2200. Please make sure you obtain the contact person's Name, ID #, and the Work Order #.
  • At times, the EOC is unable to provide a Work Order #. Please have the EOC email the following address when a work order number is obtained, [email protected]
  • The EOC will indicate if the reported issue is considered an "Emergency" and the repair will be within 24 hours. If you feel the issue should be considered an "Emergency" and the EOC does not agree, please telephone the President immediately (day or night). Additionally, if the EOC has approved the issue as an "Emergency", please telephone President Hansen.
  • All buildings and grounds maintenance damage/issues must be placed in the station log book.
  • Additionally, if no action has been taken, but you have a suggestion to temporarily fix the issue, please provide your idea below
